Proof of Consent (Opt-In)

How Users Opt In

Users provide consent to receive SMS and/or phone communications from Nick Drishinski, LLC by completing a form on our website and explicitly agreeing to receive messages by checking an unchecked opt-in checkbox. Consent is collected through the following methods:

  • Website contact forms

  • Appointment booking forms

  • Account registration forms

At the point of opt-in, users are clearly informed that they are agreeing to receive SMS messages and/or phone calls.

Opt-In Disclosure

The opt-in language presented to users is as follows:

“By checking this box, you consent to receive SMS messages and/or phone calls from Nick Drishinski LLC related to your account, appointments, or services. Message frequency may vary. Message and data rates may apply. Reply STOP to opt out at any time. Reply HELP for help.”

The checkbox is not pre-checked, and submission of the form is not possible without explicit user action.

Type of Messages Sent

Messages sent may include, but are not limited to:

  • Appointment confirmations and reminders

  • Customer support responses

  • Service-related notifications

  • Account or order updates

We do not send promotional messages unless explicitly stated at the point of opt-in.

Opt-Out Instructions

Users may opt out of receiving SMS messages at any time by replying:

  • STOP — to unsubscribe

  • HELP — for assistance

Opt-out requests are processed immediately, and no further messages will be sent unless the user opts back in.

Consent Record Keeping

We maintain records of user consent including:

  • Phone number

  • Date and time of opt-in

  • Source of opt-in (URL or form name)

  • IP address (when applicable)

These records are stored securely and can be provided upon request for compliance purposes.
